Key Benefits

  • Service Priorities based on who has the greatest need.
  • Professionally trained and courteous staff.

Assessments

Prior to receiving services, individuals must have an in-home assessment to determine eligibility (see below).

Assessments are conducted by a licensed social worker and a registered nurse.  The assessment will include developing an individual care plan in cooperation with you and / or your family.  The care plan may include referrals for services provided by other agencies as well as Clermont Senior Services.

If eligible for home care services, you will be assigned a care manager.

Your service needs will be reviewed with you periodically to determine if changes are needed.

The care manager is responsible for planning and monitoring your care and services, and is your advocate to ensure that your needs are met.
