This service provides quality Home Care assistance to older adults allowing them to remain active and independent in their homes for as long as possible. Professionally trained home health aides provide assistance with bathing, personal hygiene and other daily living needs. Homemakers do light housekeeping, laundry and run errands. Respite Care is available to 24-hour caregivers. Key Benefits
AssessmentsPrior to receiving services, individuals must have an in-home assessment to determine eligibility (see below). Assessments are conducted by a licensed social worker and a registered nurse. The assessment will include developing an individual care plan in cooperation with you and / or your family. The care plan may include referrals for services provided by other agencies as well as Clermont Senior Services. If eligible for home care services, you will be assigned a care manager. Your service needs will be reviewed with you periodically to determine if changes are needed. Care ManagementCare managers provide an organized approach to providing services to older adults, including needs assessments, development of a plan of care, referrals to other agencies and follow-up visits. The care manager is responsible for planning and monitoring your care and services, and is your advocate to ensure that your needs are met. Homemaking
